If you, the Foster Care Provider are the previous owner of the said dog make sure you fill out the Release Form. Thank you for fostering the animal until we can get him/her a home better suited to them. We soon will be giving them their vaccinations and scheduling a vet appointment to be altered. We require a minimum donation of $30.00 per released animal, however that requirement has been removed. Thank you again for being a responsible person and not just finding your pet a “free home”. We will be spending time with each animal to correct behavior problems and seeing what kind of home they need. After we have spayed/neutered or trained your animal you may not keep the animal or find him a home yourself. If you wish to do so, please feel free to fill out the adoption application. We do understand most pet owners don’t go to college to own a pet. Sometimes it just takes a little help.

The undersigned FOSTER CARE PROVIDER(s) do hereby release, discharge and hold harmless ON THE OTHER SIDE OF THE FENCE and/or any other person, firm, or corporation charged or chargeable with liability, their heirs, administrators, executors successors and/or assigns from any and all claims, damages, costs, expenses, loss of services, actions and causes of actions belonging to said FOSTER CARE PROVIDER(s) arising out of any act or occurrence from the present time, particularly on the account of the FOSTERING Of the above named dog.

CONDITIONS OF FOSTER

The above said dog is free of any liens or encumbrances except for the conditions specified in this contract.

Foster Care Provider{s} agrees to pay the full adoption fee of $110.00 in the event the said Foster Care Provider wishes to adopt said dog.

Foster Care Provider{s} agrees to pay for all supplies for said dog, including food and toys.

It is agreed that said dog would be maintained within a fenced-in yard or an approved dog kennel. During times of inclimate weather said dog will be housed indoors away from the elements. The above mentioned dog will AT ALL TIMES be under the Foster Care Provider{s} control and is never to be allowed to roam free.

It is understood that ON THE OTHER SIDE OF THE FENCE assumes NO responsibilities for damage, or injuries caused by the above-mentioned dog.

All Foster Animals should be quarantined for a minimum of 10 days - kept at home and not allowed to visit parks, pet supply stores, groomers, etc. This 10-day period allows time for illnesses to become apparent and for vaccines to take effect. We encourage foster homes to keep animals completely isolated from personal pets during the quarantine period whenever possible. Unfortunately, it is not always realistic to expect foster homes to be able to keep foster animals, especially adult dogs, completely separate from other animals in the home at all times. We do our best to educate foster homes on issues of disease transmission and trust them to make responsible decisions about how and when they allow their foster animals to interact with other animals in the home. We do, of course, insist that foster homes isolate animals that are sick or injured, or that have specific behavioral issues associated with other animals.

The Foster Care Provider{s} agrees to keep ON THE OTHER SIDE OF THE FENCE informed of any address changes. It is also understood that at NO TIME will the above mentioned dog be SOLD, GIVEN AWAY, ABANDONED, SOLD TO SCIENCE FOR RESEARCH, USED FOR ANIMAL SACRIFICES, WILL NEVER BE USED AS FOOD FOR HUMAN CONSUMPTION OR THAT OF OTHER ANIMALS, WILL NEVER BE GIVEN TO OR USED AS OR IN THE TRAINING OF FIGHTING ANIMALS. The Foster Care Provider{s} have absolutely no right to adopt said dog out. ALL parties wishing to adopt dog MUST fill out the proper forms. If someone wishes to adopt said dog, please have them contact us and we will immediately work on filling out the proper adoption forms.

It is also understood that the Foster Care Provider{s} is not responsible to pay for the regular vaccinations or vet visits scheduled by ON THE OTHER SIDE OF THE FENCE, unless specifically stated in this contract. The Foster Care Provider{s} is responsible for any and all vet bills incurred without express written permission from ON THE OTHER SIDE OF THE FENCE, unless the vet can write a letter stating the vet visit appeared to be a life threatening situation requiring immediate attention.

The Foster Care Provider{s} will inform THE OTHER SIDE OF THE FENCE of any signs of sickness or in the event of death.

Changes, additions, or any other alterations of this contract MUST BE IN WRITING and signed and agreed upon by ALL PARTIES INVOLVED.

Special clauses:

Failure to honor any part of this contract will result in a penalty of not less than $500.00 per occurrence payable to ON THE OTHER SIDE OF THE FENCE. ANY AND ALL LEGAL FEES AND COURT COSTS NECESSARY TO ENFORCE THIS CONTRACT WILL BE THE OBLIGATION OF THE Foster Care Provider{s} INCLUDING ALL THOSE INCURRED BY ON THE OTHER SIDE OF THE FENCE.

ON THE OTHER SIDE OF THE FENCE gives NO warranties covering the health or temperament of the above mentioned dog both expressed and implied.

THE ENTIRE AGREEMENT BETWEEN THE PARTIES ARE CONTAINED HEREIN

IN CONSIDERATION OF MY RECEIVING THE ABOVE SAID DOG, I HEREBY AGREE TO CARE FOR THIS DOG HUMANELY, including adequate food, shelter, water and veterinary care. I also agree to keep the dog as a house pet, to be exercised in a fenced yard or on a leash and NOT to be free to roam the Streets. I also agree NOT to sell, trade, or dispose of this dog in any way, but to RETURN the dog to ON THE OTHER SIDE OF THE FENCE if at anytime I wish to relinquish custody. I further agree that the dog shall not be chained or tied, and shall not be used for breeding. It is understood that ON THE OTHER SIDE OF THE FENCE may examine and may make inquiry about said dog at any time. If not satisfied with the condition of the dog or the conditions in which it is being kept, said dog may be reclaimed by ON THE OTHER SIDE OF THE FENCE immediately. ON THE OTHER SIDE OF THE FENCE cannot be held responsible for the conduct of this dog. All interaction between this dog and any minor child or children is the responsibility of the Foster Care Provider{s} of this dog. PLEASE BE RESPONIBLE AND NEVER LEAVE CHILDREN UNSUPERVISED AROUND ANY ANIMAL.
